The Emergency Care program provides short-term, non-medical care for residents who have
had a recent serious illness or injury or have recently returned home from a hospital until they can regain the ability
to care for themselvies or make arrangements for more permanent care. Care is limited to 40 hours per year which can include one
or more care periods during the year. The scheduling of care will be determined by the resident and a program coordinator.
There is a co-pay of $5.00 per hour.
One applies for this care by completing an Emergency Care Request form which can be obtained from the wall rack in the SCR Administration
Office or by downloading it
(Click here for Emergency Care Request). If you cannot
download this form, first download Adobe Reader
(free). The completed form is then dropped in the locked box located below the wall rack.
Only our Sunshine coordinators and the agency providing the care will have access to the information on the application. A Sunshine coordinator
will pick up the form and contact the applicant within 24 business hours to schedule a meeting to discuss the program further, answer any questions,
and help the applicant select one from four approved agencies. A representative from the selected agency will then prepare an in-depth
assessment to establish the applicant's needs and schedule the care, entering into a contract with the applicant.
The selected agency will provide non-medical care as requested, such
as preparing light meals, providing light housekeeping, laundering bed linens, and bathing.
It cannot provide any kind of medicine or medical treatments. The agency collects the $5.00/hour co-pay from the care giver and bills the Foundation for the
balance.
